When you use the contact or consultation form, we receive what you choose to share: your name, phone number, email address, your message, and any photos you choose to upload. We use it to respond to you, answer your questions, and coordinate scheduling. Please don't include detailed health history in a website form — that conversation belongs in the office, where it is protected as part of your patient record.
Online scheduling
Appointment booking on this site is provided by NexHealth, a scheduling service we work with. Information you enter in the scheduling flow is handled by NexHealth on our behalf and is also subject to NexHealth's own privacy policy.
